Cheri Crider Dental Assistance Program Application
Please complete the following form to apply for dental assistance. We will review your application and get back with you as soon as possible.
The Cheri Crider Dental Assistance Program comprises support, education and therapeutic modalities to help both male and female survivors of human trafficking in their healing journey. It is for survivors only who are 18 years of age or older and who have been trafficked in the United States.
Cheri Crider Dental Assistance Program Application Process:
1. Complete Application for Dental Assistance.
2. Once your application is submitted, you will receive an email confirmation.
3. If accepted, you will need to complete the Authorization and Release Forms. This will be sent to you separately.
4.The Board will meet within two months of your application (unless it is an emergency situation) for review and consideration.
Note: There is no guarantee that you will be approved for this program. Acceptance depends greatly on meeting the guidelines and funding available.
